Hiring FULL-TIME and PART-TIME
The Direct Support Professional Pay rate is $16.00/hour for the day rate and $15.00/hour for the sleep rate.
Note: There is a different sleep pay rate for sleep hours which are midnight - 6 a.m. sleep shift. (Sleep shift is paid at $15.00/hr; Employees sleep during this part of the shift)

SUMMARY
This position provides services and supervision levels for adults with developmental disabilities based on their individual plans.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Assures the health and welfare of each consumer served.
- Models healthy daily living skills and behavior.
- Teaches problem-solving skills.
- Establishes a therapeutic relationship with each consumer served, built on respect for each one of those consumers.
- Knows and assists in the implementation of the daily schedule of each consumer served (i.e. transports to various appointments, activities, outings, etc., which requires use of a personal vehicle and the ability to drive).
- Assists with the household routines (i.e. cleaning, cooking, laundry, etc).
- Helps consumers develop their skills during their daily living routines. Provides teaching and guidance in areas of personal hygiene, grooming, and home maintenance.
- Fosters positive relationships between consumers and their families when applicable.
- Ability to handle and implement procedures during emergencies and crisis situations.
- Completes and/or updates all necessary documentation as required, including but not limited to HPC’s, MAR’s, skill developments, and unusual incident reports.
- Must be willing and able to provide direct care services to consumers as needed or determined by the consumers’ plan or supervisor.
- Implements team decisions, working collaboratively and supportively with all other team members.
- Attends and assists the consumers participating in spiritual services and activities as requested by the consumers served.
- Responds to medical emergencies with appropriate treatment as trained.
- Professionally represents Flat Rock in keeping with the Flat Rock Mission Statement.
- Meets requirements and is able to perform all the essential functions of the position with or without reasonable accommodation.
MARGINAL FUNCTIONS
Performs other related duties as may be assigned by the House Supervisor, Service and/or Training Coordinators, Director of Community Services or President / CEO.
REQUIREMENTS
- Must be at least 18 years of age.
- Must have a high school diploma or equivalent.
- Must be able to communicate effectively with consumers, co-workers, and supervisors.
- Must have sound judgment to be able to ensure the health and welfare of consumers served.
- Must be able to read, write and complete basic reports.
- Must have the ability to problem-solve, follow consumers plans, supervisors’ directions, and able to work independently as well as part of a team.
- Must be able to lift up to fifty pounds, climb stairs, balance, bend, squat, kneel, crawl, reach and grasp.
- Ability to add, subtract, multiply, and divide.
- Completes all mandatory, on-going training each year.
- Attends all mandatory in-services and training sessions.
- Must have and maintain a valid Ohio Driver’s license and acceptable driving record.
- Must have use of a personal vehicle and maintain valid auto insurance.
- Must have a working phone number with the capability of accepting messages.
- Certification in First Aid / CPR, Medication Administration, and all other required certifications.
- Must be able to perform the essential functions and the physical demands of the position with or without reasonable accommodation.
- Experience working with persons with developmental disabilities preferred.
There are aspects of driving our company vehicles while working with our individuals therefore DSPs will have an Ohio Driver's License with less than 6 points and no more than 2 moving violations. Post offer pre-employment screening will include completing a driving record check and criminal background check. We are a drug-free workplace.

Transportation is also covered by the employee so staff is required to have a vehicle and insurance. (mileage reimbursement is paid) and acceptable driving record.
