Flat Rock continues a dedication to excellence by receiving a perfect rating from Medicaid in 2014.

We are thrilled to announce that we once again received zero citations in both the areas of Health, Program and Life Safety.

Flat Rock has received a perfect score on the Medicaid survey many times in past years. Flat Rock is able to achieve this outstanding accomplishment and remain a provider of choice in the state of Ohio because of the tireless and outstanding efforts of our employees who adhere to the highest standards of excellence set forth by the State.

Flat Rock is extremely proud of the exceptionally hard work of our staff to achieve this remarkable accomplishment. We are already preparing for next year’s audit by continuing our daily work of adhering to the high standards of quality and excellence. Through the evaluation of our mission we hope to flourish as an agency.

This legacy of excellence has been illustrated through Flat Rock’s accommodations throughout the last few years and is something that Flat Rock is exceedingly proud of. Within the last year, Flat Rock received their EAGLE accreditation from the United Methodist Association (UMA), a 3-year license from the State of Ohio Department of Developmental Disabilities and a perfect Medicaid audit.

Flat Rock would like to extend its sincere thanks once again to our staff for their outstanding work. It is because of the tremendous heart and dedication of Flat Rock’s direct support staff, supervisors, program staff, operations, leaders and other staff that Flat Rock is able to achieve this remarkable accomplishment.
