Qualified Intellectual Disabilities Professional (QIDP)
The Qualified Intellectual Disabilities Professional (QIDP) serves as a member of the Flat Rock Care Center (ICF/IID) Program Team led by the Administrator of Flat Rock Care Center.
Reports to: Administrator – Flat Rock Care Center
Location: Flat Rock Care Center located in Flat Rock, Ohio (Bellevue area; NE Seneca County). This position must be on-site with only rare exceptions. A mobile phone and laptop will be provided. Occasional, infrequent travel for meetings, events, and conferences, mostly throughout northern Ohio may be required. Mileage reimbursement is provided for company-related travel.
Schedule: Salaried, Monday – Friday, usually regular day shift office hours; however, emergency calls may be received during evenings, nights, or weekends in case of an emergency issue with a resident. Except in extreme cases, most off-hours calls can be resolved quickly by phone/email. QIDPs are primary back-up for the Administrator of the Flat Rock Care Center and may be temporarily designated as “Administrator” when the Administrator is unreachable such as for vacation travel, illness, family matters, etc. per Medicaid and Ohio Department of Developmental Disabilities rules and requirements.
Position Overview:
Oversees and directs the care, programs, and services provided to an assigned group of Flat Rock Care Center residents, ensuring quality care and services for all and compliance as an Intermediate Care Facility (ICF/IID) subject to Medicaid Certification Standards and Licensure Rules. Contributes to a high-performance, engaged, and quality-focused employee culture in the Flat Rock Care Center.
Responsibilities
Program and Service Planning:
- Oversight and Compliance:
- Oversees and directs the care, programs, and services provided to an assigned Flat Rock Care Center residents, ensuring quality care and services for all.
- Collaborates and coordinates with Director of Nursing regarding physicians in their provision of services to Flat Rock Care Center, including Medical Director, psychiatrist, neurologist, and others.
- Must be able to get results for Flat Rock Care Center by collaborating with other managers and staff under others’ direct supervision or working in other parts of the organization.
- Creates quality programs to meet needs/enhance the lives of residents while maintaining compliance with the rules and guidelines for ICF/IID Medicaid Certification Standards, state Licensure Rules, and other state and federal
laws and regulations. - In collaboration with the Administrator – Flat Rock Care Center, investigates Major Unusual Incidents (MUIs), Unusual Incidents (UIs), Injuries of Known/Unknown Origin, and allegations of abuse/neglect.
- Participates as a member of the Interdisciplinary Team for decisions regarding program services. Must be able to collaborate with an Interdisciplinary Team in coordinating the residential care and treatment of residents served by Flat Rock Care Center.
- Qualified Intellectual Disabilities Professional (QIDP) Duties: This position,
- Supports assigned residents by reviewing data/progress, revising and creating plans, including the
Individual Program Plan (IPP/Individual Service Plan (OhioISP) based on each individual’s needs, wants,
and goals. - Ensures:
- Consistency among external/internal programs and services.
- Services are designed and delivered to provide residents with an appropriate active treatment program.
- Discrepancies or conflicts between programmatic, medical, dietary, and vocational aspects of the individual’s assessment and programs are resolved.
- Follow-up on recommendations for services, equipment, or programs.
- Adequate environmental supports and assistive devices are present to promote independence.
- Required records and documentation are prepared and updated and reports for the IPP/OhioISP
process are complete. - Residents’ rights are maintained and service needs are provided as identified in the IPP/OhioISP;
- Staff are adequately trained in the active treatment process and have the skills to ensure the delivery of
quality care.
- Plans and implements quality care for assigned residents.
- Regularly spends time with assigned residents in their living unit and/or at their Adult Day Support Program, problem-solving in ongoing therapeutic treatment.
- Coordinates and -participates in the quarterly/annual review process for assigned residents.
- Works with assigned residents to create and implement IPPs/ISPs. Ensures plans are approved, documented, and reviewed and that all plans are individualized.

- Supports assigned residents by reviewing data/progress, revising and creating plans, including the
- Innovation/Sustainability:
- Seeks innovative strategies to provide highest quality services to residents and to improve operations.
- Creatively uses technology, volunteers, interns, and other methods to improve outcomes, quality care, and efficiency.
- Maximizes sustainability in operations. Seeks to improve revenue, minimize waste, and reduce environmental impact.
- Influence on Staff:
- Trains/coaches staff, parents/guardians, Board, and others regarding each resident’s IPP/OhioISP and /or program policies to ensure compliance with ICF/IID requirements. Acts as liaison between parents/guardians and FRCC for assigned residents.
- Contributes to a high-performance, engaged, and quality-focused employee culture.
Specific Function Assignment: All QIDPs are assigned to an additional functional area within Flat Rock Care Center. Examples include quality assurance, education, therapeutic
services or similar. The full job will be a combination of the QIDP job as well as the functional area assignment.
Other: Attends all mandatory in-services and training programs as scheduled. Ensures up-to-date knowledge of regulations for ICFs. Attends meetings related to the care, services, and programs for each assigned resident (i.e., house, annual, etc.). Performs all essential duties and responsibilities of the position with or without reasonable accommodation. Other duties as assigned.
Eligibility Requirements
Mandatory requirements have an * asterisk.
- Must be able to be designated by Flat Rock Care Center as a human services professional (QIDP) by having a bachelor’s degree from an accredited college/university in a human services field, including, but not limited to sociology, special education, rehabilitation counseling and psychology.*
- Must have one or more years of full-time (or equivalent amount of part-time) paid work experience in the provision of specialized services to a sizeable population of individuals with intellectual/developmental disabilities and supervision of staff.*
- Previous Preferred Work Experience:
- As a QIDP;
- With training content and effective methods;
- With positive behavioral support implementation and strategy development;
- Experience in the following software package or web-based programs would be helpful: QuickMAR, CareTracker, and OISP Salesforce.
- Previous Preferred Work Experience:
- Must be proficient at using office technology, including email, Microsoft Word, Microsoft Excel, Microsoft 365 or similar suite of products, and internet browsers.
- Must successfully pass a background investigation and consent to be enrolled in the Ohio Department of Developmental Disabilities in the Ohio Attorney General’s retained applicant fingerprint database known as “Rapback.”*
- Must be able to perform the essential duties and responsibilities of the QIDP role and the functional area assignment with or without reasonable accommodation. *
