Habilitation Specialist for Flat Rock Community Services

The Habilitation Specialist is responsible for ensuring recreational groups and programs are implemented that meet the social, emotional, cognitive and physical needs of the participants enrolled in the Adult Day Program.

Job Description:

Under the supervision of the Habilitation Specialist Supervisor and Director for Community Programs, the Habilitation Specialist supervises and/or cares for the individuals’ needs by following the applicable Plan of each individual served in the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Able to understand and implement a program which is aimed at increasing the functional levels of individuals through vocational, social and daily living tasks.
  • Models healthy daily living skills and appropriate social behavior.
  • Teaches problem-solving skills.
  • Establishes a therapeutic relationship with each individual, built on respect.
  • Follows behavior support strategies with an emphasis on verbal redirection.
  • Knows and assists in implementation of the daily schedule of each individual.
  • Follows the applicable plan of each individual served.
  • Assists in Therapeutic activities, such as physical, leisure, and recreational games and sports in the community
  • Provides teaching and guidance in areas of personal hygiene, grooming, or any identified self- care need while in the community
  • Knows how to handle and implements procedures during emergencies and crises in the community.
  • Completes necessary charting thoroughly and accurately for all community activates.
  • Maintains compliance with training and required certifications within compliance due dates. Attends all mandatory in-services, training sessions and completes all online training as scheduled by the agency and as directed by the Adult Day requirements.
  • Transports and participates with individuals in off-campus community activities, using agency vehicles or personal vehicles as needed.
  • Implements team decisions, working consistently and supportively with team members.
  • Responds to medical emergencies with appropriate treatment as trained.
  • Undergoes first aid and CPR training as scheduled.
  • Undergoes Medication Administration training as necessary to maintain certification.
  • Must be willing to provide care for any individual as needed or determined by the Director of Community Programs, Habilitation Specialist Supervisor or Administrator.
  • All duties must be performed with the highest level of integrity and support principles of person centeredness.
  • All duties will be executed in accordance with Flat Rock Community Services policies and procedures and adhere to all regulatory requirements.
  • Must be able to perform each of the essential functions and the physical demands of the position with or without reasonable accommodation.

 

Communication Skills:

The employee must demonstrate respectful and professional communication with coworkers and individuals served. This includes:

  • Verbal Communication: Using appropriate language and tone that conveys respect and professionalism.
  • Non-Verbal Communication: Exhibiting positive body language, such as maintaining eye contact, using appropriate gestures, and displaying a friendly demeanor.
  • Active Listening: Showing attentiveness and understanding through nodding, paraphrasing, and asking clarifying questions.

MARGINAL FUNCTIONS

The Habilitation Specialist is required to perform all other duties as assigned by the Habilitation Supervisor, Coordinator of Adult Programs, Director of Adult Programs or the Administrator of Flat Rock Community Services.

 

QUALIFICATIONS:

  • Must have a high school diploma or equivalent and be at least 18 years of age.
  • Must have excellent communication skills, both written and verbal. Should have basic math skills, such as the ability to add, subtract, multiply, and divide.
  • Must be able to appropriately represent a faith-based agency.
  • Must have sound judgment so as to be able to ensure the health and welfare of individuals served.
  • Must be able to problem-solve, follow individual plans and supervisors’ directions, and be able to work independently as well as part of a team.
  • Must have compassion, patience, and tolerance for persons with differing abilities.
  • Must have a valid Ohio Driver’s license and acceptable driving record.
  • Must complete all orientation and ongoing training in a timely manner.
  • Must have use of a personal vehicle and a home and/or cell phone telephone number.
  • Must have favorable references.
    Previous experience working with persons with developmental / intellectual disabilities preferred.
  • Must demonstrate a positive, professional appearance and attitude.
