Each of our programs and services help individuals live as independently as possible while providing the structure, stability, and encouragement they need to thrive.
Respite Care
Currently paused – new enrollment temporarily unavailable.
A Medicaid Waiver for respite care provides assistance for short-term stays at the Care Center. Families or caregivers may request temporary support — from a few days up to 90 days. Two respite beds (one for a male and one for a female guest) provide access to the same attentive care, services, and therapies as our full-time residents.

Life Enrichment
Life Enrichment programs provide holistic, hands-on experiences that promote wellness and creativity. Participants gain health and safety skills, engage in creative therapies, and explore new ways to express themselves.
Program Elements
- Art therapy — crafts, painting, and creative projects
- Music therapy — singing, instruments, karaoke, and talent shows
- Wellness activities — fitness, mindfulness, and social engagement
Each activity builds confidence and self-expression while celebrating every person’s unique abilities.
Day Habilitation Programs
Origins, Frontiers, and Journeys — expanding opportunities for growth.
Flat Rock’s Day Habilitation programs offer meaningful day experiences at varying stages of independence.
Origins – Flat Rock Campus
A quieter, structured environment for individuals starting to explore their world.
Activities include:
- Art, music, and movement
- Small-group social interaction
- Early life-skills training
Frontiers – Bellevue
Formerly Flat Rock Adult Advantages, this program is designed for those ready to take the next step toward independence.
Participants:
- Explore their community
- Develop daily living and vocational skills
- Strengthen relationships and teamwork
Journeys – Seybert Center
Journeys is for individuals ready for deeper community involvement.
Program Focus:
- Life-skills learning
- Recreation and community activities
- Support for employment and social engagement

Behind every Flat Rock program is a dedicated team providing administrative, financial, and development support. Flat Rock Homes ensures that each program has the staff, resources, and tools it needs to deliver exceptional care with consistency and compassion.
Homemaker and Personal Care (HPC) Services
Drop-in homemaker and personal care services for individuals with disabilities living in the local community. These services help participants manage daily living tasks with greater independence.
Non-Medical Transportation Services
Transportation for individuals attending Adult Day Habilitation programs or community employment. This service ensures participants can safely travel to and from programs, workplaces, and community activities.
