Once an individual is admitted into the care of Flat Rock Care Center, we establish a covenant relationship with the individual and family/guardian(s) ensuring that we will care for them and their loved one until care is no longer needed, the individual and family/guardian(s) choose transition out of Flat Rock Care Center or the individual’s needs can no longer be met at Flat Rock Care Center.
Many people who find a home at Flat Rock have had multiple doors closed to them.
The parents and families of individuals living at Flat Rock Care Center enjoy sharing stories of how their loved one’s life was transformed. One family shared, “Flat Rock is the best thing we could imagine this side of heaven.” Another family stated, “I gave my son birth but Flat Rock gave him life.”
Flat Rock serves individuals who have intellectual and developmental disabilities (ID/DD), with coexisting behavioral and/or psychiatric needs. Their diagnoses are multifaceted including Autism, Down’s Syndrome, Obsessive Compulsive Disorder, Bi-Polar Disorder, etc. and Seizure Disorders, along with other health issues.
At our state-of-the-art Care Center, individuals live in six interconnected homes, that can each house six residents. In these homes, individuals work toward daily individualized goals to enhance self-care and daily living skills, which are monitored by a Qualified Intellectual Disability Professional (QIDP). They eat their meals together as a family and relax and enjoy strong social relationships with those around them. This home-like setting allows staff and individuals to develop strong relationships, encouraging even those with the most significant social anxiety to build lasting bonds.
Medical care is provided onsite by licensed nursing staff under the direction of our Director of Nursing (R.N.) and our Medical Director (M.D.). Flat Rock seeks to work with health care professionals who have a special interest in persons with developmental disabilities to effectively meet individual health care needs. The medical team at Flat Rock coordinates care for our individuals among more than fifty outside specialists in pulmonology, neurology, cardiology and other medical areas in order to provide the best care for our people. Individuals also receive speech therapy, occupational therapy, and physical therapy by licensed professionals on-site and at our Flat Rock Day Program sites to develop skills fostering independence.
