Flat Rock Care Center (ICF)
Flat Rock Care Center accepts individuals as young as 6 years old, pending current individuals served, through adulthood. Currently, there is a waiting list for admission into the Care Center for both male and female individuals.
Intake Process
The intake process includes completion of application materials and an intake meeting with the prospective resident, parents/guardian(s), Service and Support Administrator, and Flat Rock Interdisciplinary Team Members (IDS), prior to placing the prospective resident on our waiting list, if appropriate.
Flat Rock agrees to provide services to persons whose needs can be met by the facility, without regard to disability, race, color, religion, national origin or ancestry, sex, age, or gender identity including, but not limited to, making reasonable accommodations to the individual’s physical, mental, or behavioral needs to the extent required by law, unless the facility demonstrates that the accommodation would impose an undue hardship on the operation of the program.

Flat Rock Community Services
If there is an opening in Flat Rock Community Services, the typical intake process for individuals requesting services in the community requires the individual to work with their local county board in scheduling interviews with prospective providers. Once interviews are scheduled, the individual, along with their team will make a decision based on the information received in the interviews.
